TERMS & CONDITIONS

The All Kids Pediatric Dentistry Membership Plan is designed to provide affordable access to quality pediatric dental care through discounted fees for services provided at our office.

Program Guidelines

• Patient portions not included in the membership plan are due at the time of service unless prior financial arrangements have been made.
• Membership benefits cannot be combined with dental insurance, other dental plans, or promotional offers.
• Membership fees are non-refundable after the applicable cancellation period.
• A digital membership card and effective date will be maintained in our office records.
• Membership benefits are available exclusively at All Kids Pediatric Dentistry and apply only to services provided by our participating dentists.
• Membership benefits do not apply to specialist referrals, hospital-based care, or services covered under medical, automobile, or workers’ compensation claims.
• Certain treatments may not be appropriate for treatment within our office and may require referral based on the clinical judgment of our pediatric dental team.
• Membership is non-transferable.

THIS MEMBERSHIP PLAN IS NOT INSURANCE and is not intended to replace dental or medical insurance. The membership plan provides discounted fees for dental services performed at All Kids Pediatric Dentistry. Members are responsible for payment of all services received. Discounts vary depending on the type of treatment provided. Membership benefits are only available at All Kids Pediatric Dentistry.

Renewal Conditions

By enrolling in the Membership Plan, you authorize All Kids Pediatric Dentistry and/or its membership administrator to charge your approved payment method for applicable membership fees. Membership automatically renews at the end of the membership term unless canceled prior to the renewal date.

Termination Conditions

All Kids Pediatric Dentistry and/or the membership administrator reserve the right to terminate membership for non-payment or misuse of the program. If the program is terminated for reasons other than non-payment, any eligible refund will be prorated based on the remaining membership term and services utilized.

Cancellation Conditions

You may cancel your membership within 30 days of the effective date for a full refund, provided no membership benefits have been used. If membership benefits have been utilized, refunds may be adjusted based on the standard fees for services received.

Cancellation requests must be submitted in writing or by email. Membership benefits will remain active through the end of the paid membership period unless otherwise required by applicable law.

Limitations, Exclusions & Exceptions

Membership discounts apply only to services performed by participating providers at All Kids Pediatric Dentistry. Discounts cannot be combined with any other dental discount program, insurance contract adjustments, or promotional pricing. Actual savings will vary depending on services provided. All fees and included services are subject to change at the time of membership renewal.

Providers are solely responsible for the professional dental treatment and clinical services rendered to patients.
